Students must complete the appropriate forms (OCPS Sports Physical) prior to being able to participate. The Sports Physical packet should be completed and submitted in its entirety (and not turned in sections). The entire Sports Physical packet should be completed by a parent/guardian (with the exception of pages 4-5 which requires a physical exam conducted by a doctor). The exam is valid for 365 days from the date it is done by the doctor, and can cover a student for all 4 sports as long as the expiration date is not before the sport season.  If a student has had a physical within the past 3 months then a doctor can transfer the information to the new form (a new physical exam in unnecessary).

Students MUST have and maintain a 2.0 GPA (C average) or above, to be eligible to participate in our athletic programs. 6th grade students are excluded from the GPA requirement for Soccer season (but still need to medically cleared before participating). 

In order for athletes to participate in daily practices and/or games, they must be present for at least four consecutive periods that day.

Checklist for Eligibility:
2.0 GPA (cumulative) 
OCPS Packet filled out completely (all 10 pages)
Doctor exam completed and signed (on pages 4 and 5 of OCPS Packet)

 All Athletic programs will stress academic success, physical fitness, character development, discipline, and good sportsmanship. Our athletes represent our school and community and are expected to be student leaders with positive behavior. Students who use alcohol, tobacco or other drugs, have behavior or attendance problems, or violate other training rules as set by their coach, may lose their opportunity to participate.
